By my signature below, I acknowledge that I am aware of, appreciate the character of, and voluntarily assume the risks involved in participating in all activities associated with swimming in the pool and participating in any party at Prairie Pool House. By my signature below, on behalf of myself, my heirs, next of kin, successors in interest, assigns, personal representatives, and agents, I hereby:
1. Waive claim or cause of action against and release from liability the Prairie Pool House, LLC, from any liability for injuries or death to my person or property resulting from my use of the facility or participation in the activity listed above.
2. Agree to indemnify and hold harmless the Prairie Pool House, its owners and employees for any claims, causes of action, or liability to any other person arising from my use of the facility or participation in the activity listed above.
3. Consent to receive medical treatment deemed advisable in the event of injury, accident, or illness during the activity on property.
4. Acknowledge that security cameras are present and I understand that the recording is for the safety and security of the individuals participating in the event. However, I also acknowledge that the footage will be reviewed in the event that damages or accidents occur.
5. I assume all risk of injury, accident, and death participating in an activity at the Prairie Pool House.
6. I will abide by and follow all rules posted in the Prairie Pool House and agree that children under the age 12 must have an adult present in the water or an appropriate lifejacket on.

7. Acknowledge that a participant under 18 years of age signing below as a minor child, a signature is required by the parent or legal guardian of the minor child to participate.
